A national specialist palliative care and advance care planning advisory service
The End of Life Directions in Aged Care Project (ELDAC) provides information, guidance, and resources to health professionals and aged care workers to support palliative care and advance care planning to improve the care of older Australians. The project website includes
- Nine toolkits for residential aged care, home care, primary care, legal matters and working together.
- There is information on a range of topics including population groups and common clinical tools
- There is also a stream of work focused on innovative technologies and a work program relating to service facilitation.
